Fixation preference assessment is a commonly used clinical method of detecting amblyopia in the preverbal strabismic infant. In this study the results of fixation preference testing were compared with Snellen acuities on 139 naive untreated esotropic adults. Sixty-nine patients (49.6%) demonstrated free alternation and no eye preference, but one patient (1.4%) was found to have amblyopia on Snellen acuity testing. Seventy patients (50.4 %) had definite fixation preference on cover test, but 25 (35.7%) of these had equal visual acuity, and a further 21 patients (30%) had only 1 line difference between the 2 eyes with Snellen acuity. Thus, 65.7% of the patients with fixation preference did not have amblyopia on visual acuity testing. This study shows that amblyopia is unlikely to be present if there is lack of fixation preference, but that, while a preference for fixation with 1 eye may not necessarily indicate the presence of amblyopia, it may suggest that the patient is at risk of developing amblyopia.
