Résumé / Abstract

Rapid cycling (the occurrence of four or more mood episodes per year) occurs in approximately 12-24% of patients with bipolar disorder admitted for in-patient or outpatient treatment. Female patients are slightly over-represented among rapid cyclers, while an association with hypothyroidism is still controversial. Treatment of rapid cycling should focus on long-term stabilization rather than on the individual episode. Although treatment with most mood stabilizers - including lithium, valproate, carbamazepine, and lamotrigine -is less effective in rapid cycling than in non-rapid cycling bipolar disorder, these agents still may result in considerable symptomatic improvement, especially when used in combination. This article reviews the clinical characteristics and risk factors for rapid cycling, as well as recent treatment studies.
