Résumé / Abstract

Introduction. Langerhans cell histiocytosis (LCH) may present in children of any age with a variable clinical picture. The purpose of this work was to describe clinical evolution, treatment, mortality, and sequels of a group of children with LCH. Material and methods. This is a retrospective and comparative analysis of a group (n =224) of children with LCH managed at the Instituto Nacional de Pediatria (a reference Pediatric Hospital at Mexico City) in the 1970-1999 period. Demographic, clinical, radiographic and laboratory data were taken from clinical charts. Central tendency tests, Cox proportional risk test, and Kaplan-Meier survival curves were used in the statistical analysis. Results. Bone involvement, particularly skull, was the most common presentation observed; 58 cases were less than 1 year of age, skin lesions were the most common findings in them. Bone marrow dysfunction was the only monodysfunction found. Patients were treated with chemotherapy (176), radiotherapy (30), and surgical resection of the tumor (25). Vinblastine and prednisone was used all over the period; toxicity was observed in 36 cases; 121 patients are alive without tumoral activity, and 40 died. Conclusions. The most common finding in infants less than 1 year of age was skin lesions; bone marrow dysfunction was the only monodysfunction observed.
