Résumé / Abstract
INTRODUCTION: The incidence of malignant melanomas is rising, although the prevalence of the disease in developing countries is still unknown. Malignant melanomas in Brazil represents 0.15% of all malignant neoplasms. TheCTPHC is a center of reference for this disease in Brazil, which is why it was chosen for this survey. OBJECTIVES: The purpose of this survey is to evaluate the status of clinical diagnosis, the prevalence and evolution of patients bearing a malignant melanoma at the Hospital do Cancer (CTPHC) in Sdo Paulo. Sampling: The survey collected information from 115 patients at the CTPHC from January 1985 to April 1987. Medical records were the main source of information. REULTS: In this study malignant melanoma was more prevalent in 40 to 69 year old Caucasian females. Most tumors were nodular, axially distributed. Breslow's and Clark's criteria of melanoma evaluation were used. The tumors with a Breslow >1.5mm in depth were predominant. After surgical removal, the 8-year survival rate was related to the initial tumor thickness: 100% of patients bearing malignant melanoma <0.76 mm in depth were still living, but only 36.4% of those bearing an initial 3.5 mm lesion survived the follow-up period (p= 0.479). Breslow's evaluation of the tumor's thickness reflected better prognosis and evolution than Clark's criteria. CONCLUSIONS: The data confirmed the importance of early diagnosis and clearly show that emphasis should be given to specific programs to alert and instruct the population on self-detection of skin lesions, since only about 10% of the cases in this survey were treated during the initial development of the tumor.
